There are a number of people providing a range of healthcare support.
The district nursing team
The district nursing team provide nursing and social care within the patient’s home. They advise on health related matters and have the ability to contact other agencies which may allow the patient, if appropriate, to stay in the community rather than in a hospital setting.
A 24 hour district nursing service is available and they can be contacted on 0300 421 6074 from 8am to 4pm for routine matters.

Midwife
The midwife provides pre-conceptual counselling, antenatal and postnatal care and gives professional advice on all aspects relating to the birth of your baby. This care is given in liaison with the doctors.
Newly pregnant patients need to self refer themselves to the midwife team as early in their pregnancy as possible, in one of two ways:

Health visitors
A health visitor is a registered nurse who has received training particularly related to babies, children and pregnant women. Their role is to provide families with children under five years old with support and advice around the general aspects of mental, physical and social well being.

Community dementia nurse
Community dementia nurses work with your GP to provide advice and support around receiving a diagnosis of dementia.
Community dementia nurses support the practice team to offer a yearly review for people diagnosed with dementia & can provide advice & support to people living with dementia & carers.
